Your first visit will include a thorough examination and consultation with Dr. Jake Hutzell. Upon arrival, you will complete your patient information sheet so that we may learn more about you and your condition.

This visit tends to generally be about 20 minutes. We are more than happy to assist you with any questions you may have about your insurance. We will take a copy of your card and verify your coverage as a courtesy. The results of your examination as well as the verification of your insurance will be ready for your review by your next scheduled visit.
